- Please be informed that the filling of the Monitoring Report of the 8th Week will be opened online through the MyGuru portal to all lecturers of the Diploma Semester 2 Session 2021/2022 (A212) from 28 February to 13 March 2022.
- Accordingly, all relevant academic staff must fill in the report so that the Head of Department/Deputy Dean (Academic & International) can confirm the monitoring report filled in the prescribed period based on the timeline on the following table:

Y.Brs. Prof./Prof. Associate Professor/Dr./Mr./Mrs. are also requested to ensure:
i. Head of Department and Deputy Dean (Academic and International) confirm the report completed by the lecturers in their respective departments (Head of Department and Deputy Dean (Academic and International) MUST fill in the comments in the comments section of the Week 8 Monitoring report before confirming the report ).
ii. The Head of Department and Deputy Dean (Academic and International) monitor the academic staff who have not yet completed this report.
iii. Improvement of the Monitoring Report by the lecturer can be made after the Head of Department gives the status of Rejected to the writing of the Monitoring report of the 8th Week until the 11th week.
iv. After March 27, 2022, the 8th Week Monitoring system will be completely shut down. Any application to fill in the 8th Week Monitoring report will not be considered.
- If Y.Brs. Prof./Prof. Associate Professor/Dr./Sir/Madam experienced any problems to fill in the monitoring report such as comments were not saved successfully or the information that has been filled is missing, Y.Brs. Prof./Prof. Associate Professor/Dr./Sir/Madam can directly make a complaint via email to: myguru@ict.edu.my and Cc to: ppa@upsi.edu.my (for the purpose of recording complaints).
- Enclosed is the QR Code attachment for the following information for the reference of Y.Brs. Prof./Prof. Associate Professor/Dr./Mr./Mrs. next:
a. Guide and Sample Reporting of the 8th Week Monitoring Report; and
b. Scale and Rubric for Completion of the 8th Week Monitoring Report.
